The e-Referrals solution is a true industry-leading initiative that helps patients receive the right care, at the right time, resulting in improved efficiency for the healthcare system. It has been designed for scalability in mind, and can be re-used in other Health Authorities across Canada and other LHINs in Ontario. The solution adheres to the pan-Canadian e-Referral standards to ensure compatibility in all Provinces.
Good patient referrals are those that send the right patient to the right service or specialist at the right time. Deviation from these principles cause serious delay in accessing specialist care and pose risks to those patients not receiving appropriate treatment in time.
Prior to the implementation of the Navantis e-Referrals solution, family physicians and specialist physicians used disparate referral management systems that lacked consistent processes and appropriate exchanges of information to make informed decisions regarding a patient’s access to specialized care and/or appropriate conservative management strategies. The Microsoft Dynamics CRM 2011 and SharePoint 2010-based solution ensures that the specialists will always have the right information at the right time to make informed decisions. In addition to SharePoint 2010 and Dynamics CRM 2011, the entire solution infrastructure was built by Navantis using Microsoft products including Forefront, SCCM, and Hyper-V.
